Community Support Specialist
Community Support Workers manage and coordinate healthcare services for clients with mental health, addictions, developmental disabilities and/or co-occurring disorders. In most cases clients have to meet a medical necessity for services.
- Conduct comprehensive assessments of clients' behavioral health needs.
- Develop individualized treatment plans based on assessments and client input.
- Coordinate care between multiple service providers to ensure cohesive treatment.
- Monitor client progress and adjust treatment plans as necessary. sessions.
- Provide crisis intervention and support for clients as needed
- Advocate for clients' access to services and resources.
- Educate clients and their families about mental health conditions and available treatment.
- Collaborate with each clients multidisciplinary team including stakeholders to ensure continuity of care is being receivedÂ
